The Department of Human Resources encourages all employees to contact the Employees’
Relations Office in case of any complaint or concern. The HR department kindly request
employees to complete this form within five working days after the incident or problem first
occurred. Human Resources Dept will contact the employee at the earliest after receiving the
complaint. Any complaint sent by hard copy or email will not be considered for further process.
